Please do not print out this application; only original applications can be processed since these forms are scanned into an automated system. If you have not already filled out your application for meal benefits (free or reduced) for the 2010/11 school year, please do so as soon as possible. You can pick up an application at the locations noted in the link below. Once completed, you can send the application in to your child’s building or District Office. Only one application per household is required. If you have already submitted your application, please do not submit another one.
